Gameshark 2 v4 cd burning

Hi, i have a "gameshark 2 v4 .rar" file. after extracting it, it showed 3 files, image.ccd (1 kb), image.sub (29.1 mb) and image.img (715 mb). what should i do now to burn it and make it playable on ps2. plz help. i m desperate.

i neeed heeellllppppp. i spent 30 hrs downloading it. any one just recomend a site from where i can get info.. :'(
