@kcajblue, if you can't see the option to change custom user titles, prehaps ask an admin to turn in on for you.
I don't have it either...
Well, the custom user title option is disabled at forum level for normal users by default. That option is usually only given out as a reward of a sort, though you can always try asking an admin via PM about it and see if you get lucky that way though.
I would guess anyone that's prominent and has given to the community in some way would get it.

I don't mind people getting one (since I only see a small handful of DCEmu UK members wanting one). Just as long as it doesn't get abused like Reputation points, I don't think the privilege would be taken away.
